Most commonly, German clinics perform surgical treatment of face scars. The choice of technique depends on patient’s age, ethnicity and skin color, but the type of scar is the key factor defining approach.

Laser resurfacing

Laser resurfacing is applied to treat minor face defects and keloid scars (connective tissue that grows between cell space and forms excessive skin layers). The scars that are caused by burning, acne and abnormal wound regeneration are “polished” with the help of infrared rays. Laser rays are absorbed by skin and inhibit the process of cell multiplication which destroys connective tissues. IPL (Intense Pulsed Light) of different kinds are used in most German clinics.

There three basic types of laser units:

  1. Carbon dioxide laser (СО2) causes extra skin coagulation and provides maximal effect as it reaches skin layers as deeply as up to 20 micrometers. However, there’s a higher risk and complications and a longer recovery period.
  2. Erbium laser concentrates tiny rays in skin layer of 1 micrometer thickness. The apparatus causes cell evaporation: old skin cells are destroyed and new skin layers gradually form. Dermabrasion

Traditional laser scar treatment (dermabrasion) implies polishing of skin: several skin layers are totally removed. However, this approach is more traumatic, and recovery period can last several weeks. Fraction abrasion removes epidermis on several fragments: wounds do not form, and minor damaged areas quickly recover being filled up with collagen. Besides, fraction resurfacing activates skin regeneration: tiny canals are left in skin, then newly produced collagen reduces visibility of scars.

Dermabrasion is preferred to reduce visibility of rough and uneven scars: rays desiccate upper layer of skin, or it is “polished” with an abrading tool. Although the scars remain, they can become less visible. Keloid or hypertrophic scars will be additionally treated with steroid injections. If the results are not satisfying, incisions are stitched.

Skin flaps

Large contracture and keloid scars are treated by transplanting skin flaps. Advances of German medicine allow for flap transplantation with the help of extracorporeal perfusion device. A surgeon takes a healthy skin flap from another body part and transplants it on the affected area. To sustain blood flow to the flap, a blood transfusion bag is connected to it: the vessels are supplied with patient’s arterial blood. Rhythmic compression sustains optimal blood flow. Venous blood is collected but not used again. After two weeks of extracorporeal circulation, the flap becomes independent and exterior blood supply is no longer required. Flap viability is checked regularly using remission spectroscopy and laser Doppler flowmetry.

Cryodestruction

This technique is applied to deal with noticeable face scars including large keloid and hypertrophic scars. Luquid nitrogen is applied topically to remove several layers of skin tissue and is usually preferred for bulging scars. Requires pretty long recovery period.
