Reconstructive Surgery focuses on the restoration of form and function. Frequently there are defects on the body surface which can be covered with a tissue transfer. Substance defects may be created after tumor operations, accidents, burns or are congenital (malformations, etc.). The backbone of Plastic Surgery, namely the Reconstructive Surgery, focuses on the treatment of such substance defects. Not only will the tissue defect be covered, but the function of the body area that was distorted through damages of the nerves, tendons or muscles will be restored as well.
Breast cancer can often lead to necessary surgical procedures that result in a partial or full breast loss (ablation). After such operations most women want to receive a breast reconstruction so that their personal body image can be restored. There are various procedures available for the reconstruction of the female breast after breast cancer. The silhouette of the cancer treated breast can be restored after cancer has been eradicated. This can be accomplished mainly with two ways. Either by using foreign-body materials, like silicon implants, or either by employing autologous compatible body tissue that guarantee a long lasting result. Before choosing and deciding for the appropriate procedure for the breast reconstruction, comprehensive information concerning all the therapeutic possibilities that modern plastic surgery has to offer should be given to the patient. The brachial plexus refers to the network of nerves of the arm. Road traffic injuries or even injuries during birth can affect and damage the brachial plexus. The interruption of the nerve fibers can lead to a dysfunction and sensibility deficits of the affected arm. From there on, everyday life limitations can lead to a pronounced life quality reduction. Our goal is to restore the functionality and sensory feeling of the affected extremity so that the patinent’s life quality is improved and a ” normal life” can be possible.
Complex wounds refer to longstanding open wounds that lead to a sustained and prolonged functional deficit and negatively affected patient life quality. Complex wounds produce chronic complaints that in the case of a simple wound would only be temporary and regressive after complete wound healing. A complex wound is not always a chronic one as well. It can be complex in its type and characteristics and can, but not always, lead through its complexity to complications that prevent the healing of the wound promoting in this way its chronicity. A wound existing more than six months is considered to be chronic. Complex wounds can have various depths. A maximal depth is reached when the underlying bones are exposed or involved. Complex wounds occur more commonly to immobilized, bedridden patients and develop on points that receive the greatest pressure (back or buttocks) to sores and ulcers. These pressure ulcers should be treated promptly since a delay could lead to osteomyelitides (bone infection) and other unwanted complications.
A facial nerve palsy is due to a lesion of the facial nerve. This condition can be idiopathic (arising spontaneously or from an unknown cause) or can be the result of an injury or a tumor growth. Unfortunately a stigmatisation of a person with a unilatelar facial nerve paralysis is not uncommon. As a result patients withdraw from their social and professional lives. Burns and scalding are classified according to their extent and depth as first-, second- or third-degree whereas superficial second-degree burns are also classified as 2A and the deeper ones as 2B. Since the depth of the burn determines the prognosis and the future appearance of the burnt area, only a specialized doctor should estimate and diagnose it. The first-degree burn or scald, similar to a sunburn, is restricted only in the outer layer of the skin (epidermis), the burnt skin is red and in some cases blisters could develop. Here long term tissue damage is rather unlikely. The second-degree burn is deeper and involves the epidermis and part of the dermis of the skin. Concerning 2A burns no surgery is necessary. The burns can be treated with local bandages and ointments and future scar formation is unlikely. In the case though of 2B second-degree burns, an surgical ablation of the burnt skin under anaesthesia is necessary.
Artificial skin or wound dressings with human amniotic membrane can be used to cover the defect and ensure a satisfactory aesthetic result. Third-degree burns destroy the full thickness of the epidermis and dermis and may also involve underlying muscles, tendons and bones. Tissue regeneration in this case is not possible, the wounds cannot heal and scars develop. In order to support the wound healing process, the burnt tissue should be surgically ablated (excised) and artificial skin or a skin transplant (for example from the thigh) applied over the wound. A proper evaluation of the burn and a successful and effective treatment plan, predisposes a wide specialization and experience in this field of Plastic Surgery. There are two main categories concerning burn treatments: the acute treatment of burns and the secondary treatment measures where the burn or scald consequences are being treated (scar removal, joint mobilization) so that the life quality of the patient can be improved.
