Beauty at any price?

Who does not dream of the perfect smile - bright white and immaculate. But beautiful teeth are not only expensive, we may also have to sacrifice healthy tooth substance for them. The dentist Dr. Henning Madsen explains why dentistry and cosmetic surgery are in the same trap today.

Every Hollywood smile today is bright white, teeth immaculately straight. Is there a trend towards beauty in Germany?

Dr. Madsen: Yes, of course, this trend exists in all wealthy countries.

What price do we pay for the impeccability of our teeth?

Well, aesthetic dentistry costs money. And these are usually services that the statutory health insurance does not carry and the private may not. But that seems to be worth a lot to many people.

If you look at tooth beautification measures, is that possible without any loss? Take, we e.g. Veneers. Before sticking, a part of the tooth substance is removed.

Veneers are wafer-thin ceramic bowls that are attached to the tooth surface with a special adhesive. In the ideal case, they are only slightly invasive for the teeth and only connected to the victim of a few tenths of a millimeter of enamel. In modern veneer techniques, sometimes you can even work without abrasion. With a crown, that looks quite different, leaving only a conical dull from the tooth after grinding.

The bleaching is always in the criticism.

That, too, is actually little or not invasive. It works with oxidants that destroy the embedded in the enamel dyes so that the enamel looks white. This is not optimal for the enamel. But it does not cause any serious damage either.

On the condition that the bleaching is done at the dentist and not at home by myself?

Of course, in the case of home bleaching treatment is not under the control of someone who knows it. If used incorrectly, it can certainly cause damage. But home-bleaching does not necessarily have to be more invasive or harmful.

In which actions would you report concerns?

I would exercise restraint in all measures that sacrifice much tooth substance. There are e.g. the strategy to compensate for crooked teeth by crowning or crazy shaped veneers the tooth position. Either the shape of the teeth is badly changed or a lot of tooth substance sacrificed - sometimes even both. It easily limits are exceeded, if you want to eliminate in this way crowding the anterior teeth. That's one thing that's just too invasive, too expensive, and ultimately an unfavorable form of care.

Yes, it is possible to eradicate narrowness orthodontically, this is not invasive, the tooth substance is fully preserved and tooth anatomy is also preserved. In the case of a crown, long-term consequences can occur, because with teeth treated in this way, the nerve often dies later and one then has to make a root canal treatment.

Are flawless teeth just for the "rich"?

Maybe that's an age issue. Earlier, so until about 30 years ago, was the caries in children a matter of course. And later in the adults, the gap teeth, so the broken teeth. Today it is quite normal that there are whole classes of twelve-year-olds, most of whom are caries free. Today, young people have the opportunity to look reasonable with good care. Therefore, the question no longer arises whether one recognizes the prosperity of the teeth.
