Emergence in the auditory nerve

Normally spontaneous electrical impulses occur in the nerve fibers and are not perceived. This so-called spontaneous activity changes when the ear is sounded. Tinnitus through stress

Stress is one of the common causes of tinnitus. A stress stimulus causes a wealth of reactions in the body. Among other things, the body releases more of the stress hormone cortisol. Causes of objective tinnitus

Tinnitus Treatment

In the case of subjective tinnitus, that is to say, noises of the ear, which are only perceived by the person concerned, the treatment is not based solely on the cause, but above all on the duration of its existence. Within the first three months (acute tinnitus), the chances of tinnitus disappearing are best. The chances of recovery for those affected are therefore the better, the sooner they go to the doctor.

If subjective tinnitus has existed for more than three months (chronic tinnitus), the ear noises have usually already become independent at this time. That is, they are not going away in the true sense. Nevertheless, there are also treatment options for chronic tinnitus. These are mainly based on diverting attention away from tinnitus and giving it another, less burdensome meaning. Many sufferers are thus able to perceive the tinnitus less and less or even not to "hear". This gives them the impression that tinnitus is gone - even if the ear noises basically continue to exist.

Important for the treatment is also whether there is a hearing loss in addition to the tinnitus. If one equates these with a hearing aid, tinnitus can often be contained or even eliminated, as noises that were "lost" are consciously perceived again.

Acute subjective tinnitus

Acute tinnitus has existed for a maximum of three months. For the standard treatment of acute tinnitus, which does not improve itself within a short time, usually includes an infusion of anti-inflammatory agents (glucocorticoids). The infusion therapy with such agents is based on the assumption that a disturbed perfusion of the inner ear is the cause of tinnitus. And as a result of the circulatory disorder, the hair cells located there would be malnourished. (The hair cells are not actually supplied with blood, they are nourished by the tissue fluid that surrounds them.)

However, as we now know, a circulatory disorder is rarely the cause of tinnitus. How effective such an infusion therapy for tinnitus in individual cases, can not say exactly. Nevertheless, it seems to work for some sufferers, even if the exact reason for this could not yet be scientifically explored. Meanwhile, this treatment is not so often used.

If the cause of the acute tinnitus is an earwax plug, the doctor can remove it immediately without pain.

Hyperbaric oxygen therapy

If infusion therapy does not improve, acute tinnitus can also be treated with so-called hyperbaric oxygen therapy. However, this form of treatment is controversial and is not paid by the statutory health insurance.

Hyperbaric oxygen therapy is based on the assumption that tinnitus can lead to a lack of oxygen in the inner ear. When staying in a hyperbaric chamber, the affected person inhales oxygen via a breathing mask. The increased pressure allows more oxygen to enter the tissues and blood, providing more oxygen to the inner ear as well. If the cause of tinnitus is a circulatory disorder, acute noises in this way can be reduced in some cases. In chronic tinnitus, however, hyperbaric oxygen therapy rarely shows any effect.

On the other hand, inhaling oxygen under normal pressure conditions makes no sense as a therapy, for example by simply inhaling oxygen through a nasal tube.

Chronic subjective tinnitus

Even with a tinnitus, which already exists for three months or longer, the doctor administered at the first treatment partially infusions with circulation-promoting agents (sometimes combined with anti-inflammatory agents such as glucocorticoids) or anticoagulants.

In addition, relaxation plays an important role in the treatment of chronic tinnitus. Those who suffer from tinnitus, the torment constant ear noises without external cause. A new, two-pronged treatment approach may now make life easier for at least some patients.

A new therapy that uses both noise and electrostimulation at the same time could relieve the symptoms of some tinnitus patients.

People with tinnitus permanently experience sounds that come from no external sound source. The causes are manifold, for example, it can be caused by extreme noise that the person was once exposed to, or a head or neck injury.

In the past, scientists have found evidence that, among other things, the disorder may be related to dysfunction in the snail nuclei in the brain. In this brain region sits a special form of nerve cells, which help us to determine, for example, from which direction a noise comes.

At the same time, these neurons are involved in suppressing tactile or vibratory sensations caused by the movement of the head or neck. In people with tinnitus, the cells appear to become hyperactive and to spontaneously fire even when there are no external acoustic signals. So they contribute to the development of annoying ear noises.

In order to put the activity of these nerve cells back on track, Shore and her colleagues developed a device that uses earphones to record specific sounds for their tinnitus via the headphones and, with the help of electrodes, light electrical impulses to the face or neck of the patient. The acoustic and sensory signals occur in a very specific time interval, which the scientists previously determined in animal experiments with regard to the greatest possible effect on the hyperactive neurons.

The researchers tested the procedure in a small clinical study on 20 patients with a special form of tinnitus: they were all characterized by the ability to temporarily alleviate their ear noises by, for example, squeezing their jaws, sticking out their tongues or tensing their necks. Shore and her colleagues suspect that those affected have intuitively found a way to influence the activity of the hyperactive nerve cells themselves.

Once the scientists had set their device for each patient individually, they taught them to install headphones and electrodes independently and use them for 30 minutes daily.

Subsequently, half of the subjects underwent therapy with noises and electric shocks for four weeks, while the other participants received a placebo treatment that did not require electrostimulation. After a break of another four weeks, the researchers finally turned the tables.

On average, the symptoms of the patients improved significantly through the dual therapy. The participants reported that the ear noises were then less loud than before. In some subjects, the volume decreased by up to 12 decibels - which corresponds approximately to the buzzing of a light bulb.

In two subjects, the tinnitus even disappeared completely. At the same time, the quality of life of the participants also increased. The severity of the impact varied from person to person. In contrast, placebo treatment produced no measurable effects.

In further tests, the researchers now want to determine the optimal length for the treatment and confirm the effect in a larger group of volunteers. They also want to find out if patients with other forms of tinnitus can benefit from the approach.
