Endoscopic breast augmentation by axillary incision
Breast augmentation is a worldwide operation and there are 3 procedures for placing the implant. In Western Europe, the underbust cut is preferred, the nipple edge section is popular. The third and also very elegant procedure is the endoscopic-axillary breast augmentation.
In this technique, the access (the small incision of about 3 to 3.5 cm) is placed far away from the chest into the upper dome of the armpit. Since there is a natural wrinkling of the skin in this area, the scar is hidden in one of these natural folds: after about half a year, the scar is almost invisible.
The peculiarity of the endoscopic-axillary technique of breast augmentation lies in the fact that we can introduce an endoscope via this access and can get a high-precision image of the interior of the breast pocket with a digital 3-chip camera. This in turn means that we can operate on the breast very precisely and without a scar.
It can be said that all patients who are eligible for breast augmentation are also suitable candidates for breast augmentation with an endoscopic-axillary approach.
This breast augmentation technique is really highly recommended because the newly formed breast not only looks beautiful but is also completely scar free. And we only get a scar-free breast through the axillary, endoscopically assisted approach. An important advantage of this technique is the flexibility in cutting.
In the Asian countries, this technique of breast augmentation is very popular and is used in about 80% of cases. In Western Europe, there are few specialized plastic surgeons who master this technique and some are from Germany.
7 myths about breast augmentation
1. The breast augmentation is extremely painful and leaves scars
That's not entirely wrong, but definitely not right. Pain sensitivity varies greatly from person to person, as does the time it takes for someone to recover from surgery. Most women who have breast enhancement with implants return to work just a few days after surgery. Slight movement is even recommended because it reduces inflammation. By contrast, excessive exercise should be started at the earliest after two weeks. Regarding the scars, the implants are inserted through small incisions (about 3 to 4 cm long) below the breast, axillary region or alveolar margin. From case to case, the implant will also be inserted endoscopically, resulting in even smaller scarring.
2. Any breast health problems that occur after breast augmentation are related to the procedure
This statement is clearly wrong! There are several studies that have examined the association between breast disease (e.g., breast cancer) and breast implants. So far without having found one. Of course, there are complications directly related to the implant, such as capsular contracture or other local complications. But that does not mean that implants can cause systemic disease.
3. Implants prevent the detection of tumors in a mammogram
Not correct! If that were the case, health authorities would have long since banned implants. However, in individual cases, it may be more difficult to perform or assess a mammogram when the woman is implanted. But for experienced radiologists this should not be a problem nowadays.
4. Implant breasts often lose their sensitivity
This is partly wrong. Normally, the sensitivity, especially of the nipple, should not be compromised after surgery. However, it may come in certain cases to a reduction of the touch sensitivity.
5. After breast augmentation, bras can no longer be worn with straps
That is partly correct. Immediately after the surgery such bra should actually be avoided. However, after the healing process is complete, such or other bras with a supportive function are beneficial.
6. You do not have to do breast examinations with implants anymore
Not correct! Regular gynecological breast examinations are a necessity regardless of whether one is implanted or not. After breast augmentation, it is even more important to have regular check-ups because self-examination by the implant may be compromised.
7. Implants must be replaced every 10 years
This statement is wrong! Today's implants usually have a lifetime warranty and are only replaced if complications occur.
Breast augmentation with XXL implants
A striking cleavage with large breasts is considered by many women as a sign of their femininity. Through a breast augmentation with XXL implants, the desire for a beautiful cleavage can be fulfilled, so that this becomes an eye-catcher.
In an XXL breast augmentation, the choice of the correct surgical technique plays an even greater role than in a normal breast augmentation, since the implants have a significantly higher weight. In order to avoid that the breasts later hang or lose shape, the implants usually need to be placed behind the pectoral muscle.
Due to the size of the implants and because the implants should be placed behind the pectoralis, XXL breast augmentation requires a high level of experience and expertise. The placement must be planned precisely so that the implants are adequately supported by the pectoral muscle, since the implants fit only partially under the muscle.
After surgery, more comprehensive and accurate follow-up is required than with normal breast augmentation. You also need to wear a support bra after surgery to support the breast optimally and provide the necessary support for the implants during healing. You should also be aware of the control appointments, in which not only the wound healing but also the position of the implants is assessed.
An XXL breast augmentation is not sensible for every woman, because the physical requirements have to be fulfilled. Inserting too large an implant in very slim women can cause visible "rippling" of the breast everywhere or the "edges" of the implant may become visible. Therefore, just in the desire for a XXL breast augmentation a detailed consultation is needed to match the wishes and ideas with the actual feasible.
Can I breastfeed after my breast augmentation?
In breast augmentation there are different access routes for the silicone implants (submammary, periareolar and transaxillary). The silicone implant is usually placed under the mammary gland tissue and below the pectoral muscle.
The glandular tissue and the milk ducts are usually not destroyed, so that the breastfeeding ability is maintained. Breast enlargement via a periareolar approach, however, can also destroy milk ducts and thus affect the ability to breastfeed.
In principle, there is no right or wrong time to have a breast augmentation done. This always depends on the personal suffering of the patient. However, in the near future, if a family planning, so I recommend to perform a breast augmentation after pregnancy.
Pregnancy can often lead to weight fluctuations and changes in one's own breast tissue, so that the result of a breast enlargement can be falsified.

2. Sustainable results
Breast augmentation with autologous fat is a very sustainable method. So every woman wants breast augmentation, a result with which she is satisfied in the long term - and this is exactly what is possible with this method. Unlike a conventional breast augmentation with a silicone implant, only a single operation without further surgery is sufficient to change the implant to achieve the desired result.

Price for breast augmentation at 10-year low for the first time since 2009
Good news for those who dream of a breast augmentation: for two years, there has been a downward trend in the cost of breast augmentation, and now average prices have fallen to a level last seen in 2009. This balance can be drawn from the long-term observation of the price history of aesthetic-plastic interventions, which myBody has been publishing as the price index "GeKIS" for 10 years now.
A breast augmentation with implants currently costs an average of 5,450 euros. The most expensive was the intervention in 2012. At that time, the average price was over 6,000 euros.
What is the cost of breast augmentation?
Breast augmentation is a very popular procedure in the field of aesthetic surgery. If breast augmentation is a medically unnecessary operation, you often have to bear the cost of breast augmentation yourself. Generally, it is assumed that breast augmentation costs in Germany, depending on the size between 4,500 euros and 7,000 euros. In addition, there are the costs for the hospital stay and the aftercare. Just the aftercare is very important in breast augmentation and must be performed unconditionally.
Breast augmentation is always associated with the use of implants. Nowadays, mostly soft pillows are used, which have either a saline solution or a silicone gel inside. The pillows can either have a round shape or look like a drop. This variant is also called anatomical implant. The shell itself is usually made of smooth or textured silicone. In general, the textured form of the pillow has the advantage that it fuses with the surrounding tissue. This prevents the risk of capsular fibrosis.
The procedure for breast augmentation is under general anesthesia. First, a skin incision is made in the lower breast crease and the implant is inserted either under or over the pectoral muscle. As an alternative to the lower breast crease, the armpit height is also available, which is more suitable because the incision can not be seen.
Another possibility is to provide access via the nipple. Most often, however, the underbust variation is chosen. After the breast augmentation, the patient must wear a firm bandage. During this time, it is very important to pay special attention to your own health. Alcohol or sleeping pills are taboo in the aftermath of the breast augmentation.
