Expert Opinion: Cataract Surgery

Cataract is one of common eyes diseases coming with aging, when crystalline lens in an eye becomes clouded and patient experiences different vision problems.

Symptoms of cataract

  • Reduction of vision
  • Blurred or unclear vision
  • Glaring (discomfort in presence of bright light including sun light and artificial sources)
  • Seeing halos around sources of light,
  • Problem of seeing in darkness.

If you have any of these signs it is better to see your eye doctor. In advanced stages it is possible to see an extended white slick on the affected eye. There are a few types of cataract as subcapsular cataract, nuclear cataract, and cortical cataract.

The age is the main reason of clouded lens. The metabolic processes of protein formation have degrading with times which could cause clumping and loss of lens transparency.

The other reasons of eye disease are:

  • injuries,
  • inflammations,
  • radiation exposure.

Additional risks factors for lens clouding includes smoking, exposure to ultraviolet, some other eye diseases, unhealthy diet, diabetes, genetic factors, and others.

There are some non-operative methods to manage but they are not really effective and mostly good only for prevention, so the main treatment for cataract remains surgery.

During cataract surgery a clouded lens is removed and replaced with clear artificial lens. It is quite simple operation and normally it can be done on outpatient base. Having surgery is always decided with a doctor. First, you need examination and, normally, only in case when you have significant damage of vision an operation could be prescribed. Please apply for checkup and initial consultation with an oculist.

Eye surgery is performed in eye hospital or clinic by eye surgeon using local anesthesia. In rare cases patient could be recommended going asleep during the operation. There many different types of cataract surgery as listed below:

Phacoemulsification: the most common modern technique with use of ultrasonic hand tool. In this case clouded lens is emulsified by ultrasonic vibration.

Extracapsular cataract extraction (ECCE). During this operation almost the whole part of natural lens is removed but its elastic capsule stays untouched. The lens is removed via big incision in sclera.

Manual small incision cataract surgery (MSICS). It is more advanced modification of ECCE when clouded lens is elicited through a scleral tunnel.

Intracapsular cataract extraction (ICCE). The technique when the entire lens is removed together with the elastic capsule.

The last operation have a higher risk of complications and rarely used in the countries with developed technologies.

After removal the cataract from the eye done with help of one of the above listed methods, in most cases, intraocular lens implant (IOL) is inserted.

There are two main IOLs types:

  • Foldable IOL are used for MSICS as they can be inserted via small scleral tunnel. They are made of silicone or acrylic.
  • Hard IOL are intended to be introduced into the elastic lens capsule via bigger incision at ECCE. As a rule, they are made of polymethylmethacrylate.

At latest time, some new types of IOL are developed with additional functions which are not only replace defected natural lens but allow to correct vision in different ways:

  • Aspheric IOLs
  • Toric IOLs
  • Accommodating IOLs
  • Multifocal IOLs
